Ordering

Equipment and home modifications are ordered and supplied in multiple ways. This depends on the item or home modification type and which organisation the prescriber is from. All equipment and home modification requests must be documented either online or via a paper prescription form.

You can order equipment for DHS Equipment Program and Commonwealth Home Support Programme (CHSP) clients using iApply - Equipment Requests.

DES Online Equipment Catalogue

DES Online Equipment Catalogue

Domiciliary Equipment Service (DES) maintains an online catalogue displaying a broad selection of quality equipment which is readily available in the warehouse. The catalogue can be browsed by any user via DES External Online Equipment Catalogue.

DES range of readily available equipment items have been selected after an evaluation process and are normally available for immediate supply. Occasional supply issues may require DES to substitute another item in the instance that the exact item is unavailable. The substitute supplied may differ slightly to the item pictured on the catalogue, however will have no impact on the intended purpose of the product. If the only item available is significantly different, then a DES staff member will contact the prescriber to confirm suitability.

Equipment needs to be ordered using a prescription form.  Select the correct form, depending on the funding that is being used for the supply of the items.

You can order equipment for DHS Equipment Program and Commonwealth Home Support Programme (CHSP) clients using iApply - Equipment Requests.

Home Modifications need to be ordered using a prescription form.

Additional specifications may be required for more complex prescriptions, and these should be provided to DES using the relevant specification forms in addition to a prescription form. This enables DES to ensure that the appropriate item is provided to meet an individual need.

All prescription and request forms can be faxed to DES on 1300 295 839 or emailed to des.frontdesk@sa.gov.au.

Imprest

Imprest

Allied Health Professionals from Silver Chain RDNS SA have access to on-site satellite imprest stores at a number of sites which hold a small range of equipment types that are frequently supplied.

Forms to document supply of these items can be found within the imprest and must be completed to record the provision of the equipment.
